Application for Provisional Certificate and Official Transcript
** Students who have completed all the required courses of the curriculum are eligible to apply. Issuance of Certificate or Transcript requires approval/clearance from several authorities. After final approval by the respective Department Chairperson, the Controller of Examinations office requires an additional three (3) working days to complete the processing.
